What happens during my first appointment?

During the appointment, you and your clinician will discuss your symptoms, health concerns, medical, family, social and psycho-spiritual history, childhood history, diet, exercise and sleep habits, along with any other information that will help create your treatment plan.
Your clinician will also review any previous lab testing and/or medical records from your primary or speciality clinicians – it is best to send this information prior to your initial consult. At the end of the visit, the clinician will determine which laboratory tests you should proceed with and suggest basic changes in your diet and nutritional supplementation.


Are your services covered by OHIP or insurance?

Functional Medicine is presently not covered under OHIP.
Some insurance carriers may cover medical services and laboratory tests performed by our clinicians. We can provide a detailed receipt for services performed for you to submit to your insurance carrier directly.


How do I become a patient?

To get started on your healing journey, you can reach out to the office via email or phone. Our Functional Medicine provider will call you for an initial 15-minute phone intake to gather more information regarding your health concerns and to ensure that the care we provide fit your health needs.


What do I need to do before my appointment?

Live For You will send you intake forms and important documents for you to complete prior to your initial appointment. The clinician will also request all pertinent files to review prior to your appointment.


Can the clinicians at Live for You be my primary care provider?

No, we require that you have your own primary care provider. Establishing and maintaining a relationship with your primary care provider is important and highly recommended. We work in conjunction with your prescribing doctor who will decide when and if the patient can reduce or eliminate any medication. We will work with you closely as consultants in preventive, nutritional and functional medicine to help you address the root causes of chronic health problems.


How are my appointments conducted?

Live For You offers all sessions via telemedicine.
